1. Prospective Egg Donor Questionnaire: If you are accepted into one of our programs, potential recipients will review your responses to these questions as they consider whether you are the right egg donor for them.
2. FDA (Food & Drug Administration) Questionnaire: This consists of 50 YES/NO questions, required by the FDA. The FDA Questionnaire will not be shared with intended parents; this is only for our records.
3. Photo Upload: At the end of this application, we ask you to submit 10-15 photos of yourself from all stages of your life: infancy, childhood, teenage, and adulthood. You should also submit at least 1 current full-body photo of yourself. Any photos of your family and siblings are good too. These photos will be included on your online profile for intended parents to look at. Please make sure they are appropriate. Parents like to see photos from graduation, extra-curricular activities, and anything that gives them an insight into who you are.
4. Introduction Video: Lastly, we require you to upload a quick introduction video (about 60 seconds) about yourself before being uploaded to our database. By describing your personality and more about who you are, intended parents will be able to get to know you a little better and it will likely increase your chance of getting chosen. Please refrain from mentioning any identifiable information such as name, specific city, etc.
